New Community Pathways Webpage and E-mail Address for Questions: Below is the link to the revamped webpage and designated e-mail address for Community Pathways that includes information about the waiver renewal that was approved by the federal government on March 26, 2014. The Community Pathways Waiver provides services and supports to individuals, of
any age, living in the community through licensed provider agencies or
self-directed services that are funded by DDA. All individuals participating in
the waiver must meet the eligibility criteria for the waiver which are included
in the agreement with CMS and noted in regulations. Please note that the old New Directions Waiver - self directed services is now included in the waiver renewal.
